Our service offers specialist drug and alcohol doctors, nurses and allied health staff who respect the client’s right to choose from a range of treatments offered with our services underpinned by a philosophy of harm reduction.A Permanent Full time opportunity has become available for a Counsellor Level 1/2 , Occupational Therapist Level 1/2 or Social Worker Level 1/2 to join the Nepean Hospital team.The Clinicians in the Assertive Community Management team will provide direct frontline services to Drug and Alcohol patients who experience a range of comorbid physical and mental health problems as well as multiple social issues. The core functions of this position are to promote the principles of harm minimisation and a recovery model of care from drug and alcohol dependency, participate in developing linkages and referral pathways across a range of community-based support services and to participate in developing clinical procedures for the smooth running of the team in consultation with managers and other staff.The successful candidate will participate in meetings for the team under the direction of a medically led multi-disciplinary team, perform specialist clinical services to patients including comprehensive drug and alcohol assessments, develop and conduct timely periodic review of care management plans in collaboration with patients, and assist patients with achieving their goals. You will Provide assertive follow up for patients including undertaking home visits. Current membership (or eligibility) with the Australian Association Social Workers (AASW) or AHPRA registration as an Occupational Therapist.
- Demonstrated clinical skills in assessment, planning and evaluation of psychosocial interventions with adult patients including exercising independent professional judgement on routine matters.
- Demonstrated experience with using advanced reasoning skills when undertaking a patient’s assessment of cognitive capacity and their complex issues, report writing, developing and reviewing care plans, delivering case management activities and psychosocial interventions.
- Demonstrated experience and commitment to professional and ethical boundaries in consultation with a medically led multi-disciplinary treatment team.
- Knowledge and understanding of the social and emotional needs of patients with drug and alcohol issues including clinical response and management of trauma, grief and loss, domestic and family violence and child protection.
- Knowledge of and commitment to working and partnering with Aboriginal patients, families and communities as well as improving the health outcomes for socially disadvantaged groups of patients within the local community.
- Current Driver’s Licence and willingness to use for work purpose travel.
