They draw on evidence-based frameworks to deliver dynamic case management practice with people who have complex needs and those who have committed serious offences.You will help a caseload of complex and high risk clients meet their court order conditions and connect them to community programs and services. You will also lead and mentor staff, guiding them to deliver quality evidence-based case management practice to clients.Some of your day-to-day responsibilities include:
- conducting risk assessments
- preparing case plans
- providing interventions
- coordinating service providers and care teams
- address the underlying causes of offending
- leading and developing case management staff.
